Solera Health, Inc.
Vice President, Emerging Care Models
Teresa N. Clark, MPH, MBA, is Vice President, Emerging Care Models for Solera Health, Inc, a private equity backed medical technology company focusing on consumer digital health.  Clark carries the dual perspectives of healthcare payment innovation and population health in her leadership of the design, launch, and scale of breakthrough care models, including Solera’s award-winning lifestyle change platform.  Disrupting the current clinician-knows-best approach, Clark designed and took to market a 3-sided health platform that allowed inclusive consumer choice, a community and digital health network, and a value-based payment model for private health insurance.

Prior to joining Solera, Clark served as Vice President, Office of Clinical Affairs for the Blue Cross Blue Shield Association (BCBSA), a national federation of 36 independently operated Blue Cross and Blue Shield companies that provide the health insurance coverage for 1 in 3 Americans. In her national system-wide leadership role, Clark led clinical strategy and innovation that resulted in the profitability of several care products, including the Community Health Management Hub (CHM Hub®) and the launch of the social determinants of health partnership with the BCBS system, Lyft, and Walgreens.
Prior to joining BCBSA, Clark held leadership positions in both the health care provider and government payer sectors. Within the provider sector, Clark provided leadership as vice president, Clinical Business Strategies, for Vizient, Inc., where she designed and scaled the Health Engagement Network (HEN) for hospitals, which resulted in sustainable decreases in patient harm while increasing hospital financial performance on the HCAHPS, Hospital Consumer Assessment of Healthcare Providers and Systems. Within the government payer sector, Clark provided leadership as the Special Assistant, Value-Based Purchasing at the Centers for Medicare & Medicaid Services (CMS) under President George W. Bush’s Administration.

Clark holds a Bachelor of Science from the University of California, San Diego, with a focus on neuroscience and computer science. Clark also holds a Master of Public Health from the Johns Hopkins University Bloomberg School of Public Health, and a Master of Business Administration from the University of Pennsylvania Wharton School of Business.

Clark currently holds two patents for the BCBSA’s Community Health Management Hub (CHM Hub®), a utility patent for software development and a design patent for the consumer experience (USPTO D871,427).
